Position Overview

We are currently seeking a highly organized and compassionate Remote Discharge Planner to assist with patient discharge coordination, care transition support, medical documentation, and communication between patients, families, healthcare providers, case managers, and community care resources.

This role is ideal for individuals interested in healthcare administration, patient coordination, case management support, medical office operations, and continuity-of-care services. As a Discharge Planner, you will help ensure patients receive the proper instructions, referrals, follow-up appointments, and support needed after leaving a hospital, clinic, rehabilitation center, or care facility.

Responsibilities

  • Assist with coordinating patient discharge plans in collaboration with healthcare providers, case managers, nurses, social workers, and administrative teams
  • Support patients and families by explaining discharge instructions, follow-up steps, appointment requirements, and care coordination processes
  • Schedule follow-up appointments, referrals, home care services, transportation support, or community resource connections as needed
  • Maintain accurate discharge documentation, patient records, care notes, referral forms, and administrative updates
  • Communicate with hospitals, clinics, rehabilitation centers, long-term care facilities, pharmacies, home health agencies, and provider offices
  • Track discharge timelines, pending documentation, care transition tasks, and follow-up requirements
  • Assist with insurance verification, authorization support, service coordination, and patient information updates when required
  • Ensure discharge plans are completed accurately, professionally, and in accordance with healthcare privacy and compliance standards
  • Respond to patient, family, provider, and internal team inquiries through phone, email, and virtual communication platforms
  • Support care coordination workflows, reporting, scheduling, and administrative follow-up
  • Maintain confidentiality of patient information and follow healthcare documentation standards
  • Perform additional patient coordination and administrative duties as assigned

Qualifications

  • High School Diploma or equivalent required
  • Previous experience in healthcare administration, discharge planning support, patient coordination, case management support, social services, medical office administration, or customer service preferred
  • Strong verbal and written communication skills
  • Proficiency with Microsoft Office, Google Workspace, EMR systems, scheduling tools, CRM systems, and virtual communication platforms
  • Accurate data entry, recordkeeping, and documentation skills
  • Ability to work independently and manage priorities in a remote environment
  • Reliable internet connection and professional remote work setup
  • Knowledge of healthcare terminology, care transitions, patient records, insurance coordination, or community healthcare resources is an asset but not required
